Qualifications in becoming a Direct Care Worker: 

  • Must be 18 years of age

  • Employee Application

  • Driver’s License / Identification Card

  • Social Security Card

  • First Aid Certification (NO web-based training)

  • CPR Certification (NO web-based training)

  • Fire Safety Certification

  • Food Handlers Permit (blue card)

  • DPS Fingerprint Clearance Card

  • 10-year Navajo Nation Background Check

  • DCW Training Certification: Level 1 - Fundamentals

    Level 2 - Aging & Physical Disabilities

    Level 2 - Developmental Disabilities

    (We do in-house DCW training, if needed)

****All certifications must be current and non-expired. 


Qualifications in becoming a Non-Emergency Medical Transportation Driver: 

  • Must be 26 years of age

  • Valid AZ Drivers License

  • DPS Fingerprint Clearance Card

  • Pre-Employment Drug Test

  • Department of Motor Vehicle 5-year record

  • First Aid / CPR Course Training

  • Navajo Nation Background Check

  • AZ State / Federal Background Check

  • AZ Department of Transportation Physical Exam

  • Defensive Driving Course Training

  • HIPAA Training

****All certifications must be current and non-expired.
