We provide one-on-one care for individuals who can no longer do the things they used to in the comfort of their own homes. Our Direct Care Workers are trained and committed to provide the best services anyone can offer. Majority of our valued members are of the elderly population who still have a lot to offer with their knowledge and experience and in their presence we have so much to gain by caring for them. We encourage not only our staff but our community to talk with our elders and listen to what they have to say and if we take this opportunity to learn, we will all be enriched by the experience. 

 

What eligibility requirements are needed to receive In-Home Care? 

  • The individual who will be receiving services must be home bound or normally unable to leave his or her home unassisted. Also, needing assistance with daily living such as light housekeeping, bathing, dressing, grooming, meal preparations and medication reminders.

  • Must be an enrolled member of the Arizona Health Care Cost Containment System (AHCCCS).

  • Must apply and be approved with the Arizona Long Term Care Service (ALTCS).


To Be Eligible for ALTCS You Must:

  • Be a US Citizen or Legal Permanent Resident

  • Primarily Reside in Arizona (Temporary Absence is Permitted)

  • Be Financially Eligible

    • $2,000 Counted Asset Limit (Single)

    • $2,313 Monthly Income Limit (Some Forms Excluded)

    • Married Rules May Allow for More Assets and Spousal Income

    • SSI recipients are automatically financially eligible

  • Be Medically Eligible

    • You Must Need a “Nursing Home Level of Care”

    • Or Be an Eligible Developmentally Disabled Child

  • Have an Appropriate Living Arrangement

    • You Cannot be Approved While in the Hospital

    • You Cannot be Approved While in a Non-Certified Facility

    • You Can Reside in an ALTCS Certified / Registered Facility, such as:

      • a Skilled Nursing Facility (SNF or "Nursing Home")

      • an Assisted Living Home or Center (Including Memory Care)

      • an Adult Care Home (Group Home)

      • a Behavioral Health Home

    • You Can Also Live in Your Own Home

    • You Cannot Be an Inmate of a Penal Institution

  • You Must Have a Valid Social Security Number

  • You Must Submit a Valid ALTCS Application

  • You Must Provide all Requested Verification Documents Within Ten Days

    • Financial Account Statements

    • Written Explanations for All Transactions

    • Life Insurance Statements

    • Marriage and Birth Certificates

    • Pay Stubs or Income Statements

    • Health Insurance Cards

    • Utility, Mortgage & Unreimbursed Med Expense Statements

    • Home and Motor Vehicle Documents

    • Anything Else ALTCS May Request

  • You May Be Required to Apply for VA Pension Benefits

    • If You Are a Wartime Veteran

    • or the Surviving Spouse of a Wartime Veteran
